Raj D. Tolat, MD, MBA

Dr. Raj Tolat is a physician who specializes in Physical Medicine and Rehabilitation (PM&R).  Born and raised in New York, he graduated from The Mount Sinai School of Medicine in 1996 and completed his residency training in PM&R at Long Island Jewish Medical Center in 2000, where he served as Chief Resident in his final year.  He then completed a Master's in Business Administration with a focus on  Healthcare Management in 2005. From 2000–2008 he was full-time faculty  at Northwell and served as Residency Program Director in PM&R from 2005– 2008. In 2008, he stepped down from full-time faculty to pursue private practice, remaining on as a voluntary attending at Northwell. He went on to  build two successful practices spanning three office locations in Valley Stream, Bellerose, and Medford over 17 years, which he sold in 2025.  Dr. Tolat is Board Certified by the American Board of Physical Medicine and Rehabilitation and he brings over 25 years of clinical, academic, and administrative experience to New York Sports and Joints.
