Please call the office to schedule an appointment. If you are unable to make your appointment, please call in advance so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. If we are a participating provider of your insurance company, we will bill them for your visit and will help in any way we can to assist you in handling claims.
Please request all prescriptions and authorizations during our regular office hours. Renewals requested at other times will be filled only for extreme or emergent circumstances.

We accept Worker’s Compensation, No Fault cases and most major commercial health insurances on an Out-of-Network basis. We do not accept Medicare or Medicaid.

Some patients require medical clearance prior to their scheduled surgery. This must be done no more than one month before surgery but no less than one week prior to surgery unless otherwise indicated. Medical clearances depend on your age and medical history which may include a history and physical, blood tests, EKG and a chest X-ray.
Apply R.I.C.E. therapy, apply ice or limit activity for a smooth and a quick recovery.
Do not remove the dressing of the incision site, take appropriate pain medications prescribed by the provider.
