Insurance Information

We accept Workmans Compensation, No Fault cases and most major commercial health insurances out there.

We don’t accept Medicaid!

Appointments

Please call the office to schedule an appointment. If you are unable to make your appointment, please call in advance so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.

Fees & Payments

We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. If we are a participating provider of your insurance company, we will bill them for your visit and will help in any way we can to assist you in handling claims. However, we request that patient co-payments be made by cash or check.

Prescriptions & Renewals

Please request all prescriptions and authorizations during our regular office hours. Renewals requested at other times will be filled only for extreme or emergent circumstances.

Insurance

We accept most insurances on an Out-of-Network basis and these select plans on an In-Network basis:

We do not accept Medicaid Managed Care Plans.

With the exception of Medicaid managed care plans, we accept all commercial insurance plans on an Out-of-Network basis.

Pre-operative Surgical Medical Clearance

Some patients require medical clearance prior to their scheduled surgery. This must be done no more than one month before surgery but no less than one week prior to surgery unless otherwise indicated. Medical clearances depend on your age and medical history which may include a history and physical, blood tests, EKG and a chest X-ray.

Preparing for Surgery

  1. Do not eat or drink anything after midnight the night before your procedure.
  2. No alcohol or smoking at least 24 hours prior to your procedure.
  3. If you develop cold, fever or chest congestion prior to surgery, contact the surgical coordinator for rescheduling.
  4. Prior to your procedure a nurse or a member of clinical staff at the surgery center will contact to review a list of any and all medications you are taking.
  5. Please wear comfortable loose clothing and remove all metal jewelry, makeup, etc.

Post operative needs & expectations

Apply R.I.C.E. therapy, apply ice or limit activity for a smooth and a quick recovery.

Do not remove the dressing of the incision site, take appropriate pain medications prescribed by the provider.
